Overview
Licensed Practical Nurse (LPN) – Long Term Care, Geneva, NY. Travel - Temporary Assignment beginning 08/25/2026 for 27 weeks. Weekly pay range: $1,240–$1,304 per week. Guaranteed hours: 0 per week. Shift: Flex. Setting: Skilled Nursing Facility.

Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under supervision
- Collect data on residents’ physical and psychosocial status and communicate findings to the RN and the interdisciplinary team
- Contribute to development of an age-appropriate, culturally relevant plan of care reflecting nursing diagnoses and interventions
- Involve the resident, family, and healthcare team in care planning; revise plans as outcomes are evaluated
- Collaborate with physicians, RNs, and other staff to ensure safe, effective care and timely referrals when needed
- Maintain current nursing knowledge and competencies; participate in continuing education and mentoring of staff as assigned

Requirements
- Active New York LPN license
- BLS certification (AHA or RQI)
- Minimum 1 year of LPN experience in a long-term care setting
- Ability to care for adult and geriatric residents
- Capacity to work in a skilled nursing facility environment under supervision of an RN
