Welcome to Montage Health’s application process!Job Description:Position SummaryUnder the leadership of the department director, the inpatient Heart Failure Program Manager is responsible for the development, implementation, and management of the heart failure program across the continuum of care using a multidisciplinary approach to patient centered care. The role ensures evidence-based, high-quality, and cost-effective care for heart failure patients while improving clinical outcomes, reducing readmissions, and supporting organizational goals related to quality metrics, regulatory compliance, and patient experience.Key responsibilities include program development and oversight, clinical quality and outcomes,care coordination, data and reporting, education and training, regulatory, community outreach and growth.Skills and competencies include strong leadership and interdisciplinary collaboration skills,knowledge of heart failure clinical guidelines and disease management, knowledge of quality improvement methodologies (e.g., Lean, Six Sigma), data analysis and improvement expertise,excellent communication, organizational, and project management skills, ability to drive change and influence clinical practice, strong interpersonal skills, and ability to manage direct reports.Experience3–5 years of cardiovascular or heart failure clinical experience requiredPrior program management, quality improvement, or leadership experience preferredExperience with data analysis and performance metrics strongly preferredTeam/workgroup participation experience preferredDemonstrated leadership, problem-solving, and critical thinking skillsFamiliarity with regulatory standards preferredEducationBachelor’s degree requiredMaster’s degree (MSN, MHA, or related) preferredLicensure/CertificationsCurrent California RN licenseHeart Failure certification (e.g. CHFN) within 6 months of hireBasic Life Support (BLS) requiredAdvanced Cardiac Life Support (ACLS) preferredEqual Opportunity Employer#LI-AC1Assigned Work Hours:Full time (exempt)Position Type:RegularPay Range (based on years of applicable experience):$77.64to$103.84 #J-18808-Ljbffr
