Callen-Lorde is seeking a Primary Care Provider (NP/MD/DO) who specializes in HIV care to join our collaborative, multidisciplinary team. Clinical Care Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan. Provide care that is respectful, compassionate, unbiased, and consistent with evidence-based guidelines. Deliver care in scheduled 20-40 minute appointments and ensure timely follow-up on patient needs, test results, and referrals. Document all care promptly in the electronic health record (EPIC). Provide primary care services. Collaboration & Coordination Make appropriate internal and external referrals. Work effectively within a team-based care model, collaborating with other healthcare professionals to ensure coordinated patient care. Quality & Professional Development Participate in quality improvement initiatives and case conferences. Engage in ongoing professional development opportunities, including continuing medical education. Per Diem Rates Per Diem Day: $400/session Per Diem Evening: $500/session Per Diem Clinic Full Day: $750/session Per Diem Clinic Half Day: $500/session Work Schedule Callen-Lorde is open Monday through Saturday. This is a per diem position and we are seeking Saturday coverage. Qualifications New York State Certification/Licensure (MD, DO, or NP) Current NYS Registration and DEA Controlled Substances Registration Certificate Current CPR Certification Board certified or eligible in Family Medicine, Internal Medicine, or Infectious Disease Experience in community-based primary care preferred Skills That Will Help You Succeed Ability to work effectively with individuals from a wide range of backgrounds Commitment to accessible, patient-centered primary care Strong collaboration skills in a team-based model #J-18808-Ljbffr Callen-Lorde Community Health Center
