Infusion RN - In Home & In Suite

Coordinate and provide intermediate direct patient care in patients’ homes or alternate infusion suites Initiate, develop, and implement nursing plans of care and evaluate patient progress Perform treatments, administer medications, and educate patients and families/caregivers Assess patient needs and physical status during skilled visits and revise treatment plans as needed Obtain and clarify physician orders and report significant changes in patient condition Provide training and mentorship to Infusion Nurse I’s Teach patients and families using a patient-centered approach to support independence with prescribed therapy Communicate with clinical teams, medical providers, patients, and caregivers to ensure continuity of care Prepare real-time clinical documentation and submit it within established timelines Coordinate discharge planning and prepare discharge summaries Comply with agency policies, professional standards, regulations, guidelines, and accreditation standards Use computers and automation technologies to validate orders, document care, and follow visit schedules Respond to patient requests and initiate emergency procedures when necessary Safeguard confidential patient and proprietary information in accordance with HIPAA Explain Option Care’s services and related financial obligations to patients Complete training, in-services, and continuing education Provide oversight and coordination of paraprofessionals in the home setting Participate in multidisciplinary conferences, precepting, training, mentoring, and nursing department on-call responsibilities Insert PICC/Midlines per physician order when applicable Troubleshoot/declot obstructed central line catheters when applicable Monitor PICC/Midlines and educate on line infection prevention and routine care when applicable Requirements Active and unrestricted RN license in the state of practice Minimum of 1 years of nursing experience Current CPR certification required Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs Advanced certification and training as applicable Required licensure to operate a motor vehicle in the state of practice Access to a vehicle for business travel with proof of liability insurance Basic knowledge of computer operating systems and software applications Ability to lift up to 50 pounds with a maximum lifting of 75 pounds Willing to travel up to 100% of the time for business purposes 1-2 years of previous infusion nursing experience preferred Bachelor of Science in Nursing from an accredited school of nursing preferred Core Competencies Demonstrates expertise in patient care coordination, infusion nursing, and clinical documentation while ensuring compliance with healthcare regulations and standards.
