Catholic Community Services of Western Washington and the Archdiocesan Housing Authority (doing business as Catholic Housing Services and Coordinated Care Agency) are outreaches of the Catholic Church in Western Washington. The Counseling, Recovery & Wellness (CReW) Program is a licensed community mental health and substance use disorder treatment program and provider of the King County Integrated Care Network (KC-ICN). Our collaborative team focuses on serving adults with mental illness and/or substance use disorders to help improve individuals’ quality of life. Range of services includes diagnostic assessments, individual therapy, peer support, case management, group counseling, skills training, care coordination, medication management, and psychoeducation. Services offered emphasize building collaborative relationships with the persons served as the first step towards lasting engagement in treatment. Our team was created out of the need for onsite supportive services in our agency housing program and we adapt our services to best fit the unique needs of our clients in a variety of locations within Seattle downtown and greater King County. If you are looking for meaningful work, make a tangible difference, and drive positive change, find out more at: How YOU can make a difference: In a behavioral health setting, a nurse will be part of an interdisciplinary team and work in collaboration with psychiatric and addiction medicine providers, case managers, therapists, peers, and supervisors. The primary focus of this nursing role is the identification and engagement of individuals who would benefit from medication interventions for substance use disorders, with a special focus on Opioid Use Disorder. The nurse will additionally assist with the screening and monitoring of health conditions that can create barriers to goal obtainment and recovery. In addition, the nurse will play an integral role in treatment planning through identifying resources that improve individual health and wellness and supporting client engagement with those resources. This work involves provision of direct client care and a high level of collaboration with teammates on both a wider programmatic level and within a smaller MOUD-focused team comprised of a provider, nurse and outreach case manager. In addition, the nurse will support team efforts to provide education and training to staff and clients on substance use disorders, treatment of those disorders, and associated health complications from substance use. Due to the evolving needs of the MOUD program and its clients, the nurse in this position will need to feel comfortable providing services in both a traditional office-based setting and clinical spaces within day centers/shelters and supported housing buildings. Responsibilities Health assessment, screening and coordination Carry out health screenings focused on substance use disorders and related health conditions Determine clinical needs and levels of care where referrals may be needed. Assist clinicians and medical team in screening for common medical concerns such as diabetes, hypertension, obesity, etc. Assist clients on obtaining medical lab results and metabolic panels as needed. Supervise and periodically review quality of services provided to patients. Assist with obtaining records from outside primary care providers to receive current medication lists and report of health concerns. Assess patient physical health needs, with particular attention to wounds, applying appropriate first aid as indicated within nursing scope of practice and referring out to appropriate providers as needed. Provider supports Assist with scheduling appointments for follow up with CReW psychiatric prescribers, review charts for discharge orders, outside provider medication lists, lab results. Communicate medication issues, recent hospitalizations, and new health conditions to the treating prescriber and clinician as needed. Obtain vitals (height, weight, blood pressure, heart rate) from clients prior to their appointments with psychiatric providers and track this information in the medical record. Facilitate or assist telemedicine appointments between the client and prescriber as needed. Coordinate with pharmacies to order medications, communicate medication changes, and verify prescriptions written by CReW providers that are sent directly to pharmacies. Review refill requests for accuracy, respond to questions on orders from pharmacy and troubleshoot challenges in any refill issue between prescriber and pharmacies. Medication Management Perform administration of oral and injectable medication, primarily long-acting injectable buprenorphine, following the prescriptive authority of agency psychiatric and addiction medicine practitioners. Follow DEA regulations and CReW policies and procedures for receiving, administering and wasting medications. Monitor medication side effects and provide feedback to providers and clinicians on appropriate follow-up. Assist with enrolling clients in new pharmacies that support weekly mediset or bubble packaging. Health education for clients and staff Facilitate groups, trainings and workshops to staff and clients on overdose prevention, available pharmaceutical interventions for substance use disorders, and other topics related to decreasing negative health impacts from the use of various drugs and alcohol. Provide and ensure interventions that enhance patients’ strengths and increase their involvement in valuable activities in the community. Provide education to clients and team on medications: common side effects, counter-indications, drug interactions Care Coordination Assist clients and treatment team with referrals and access to primary care, specialty care, lab work when needed. Communicate with outside medical providers to update on medications prescribed by CReW providers and obtain medical records from the medical providers for treatment team to review. Collaborate with outside agency nursing staff located in shelter or housing projects to identify additional barriers to treatment for residents. Maintain a focus on strengths, needs, and creative solutions. Utilize the consumer’s expertise in problem solving and solution seeking. Work interdependently with others toward common goals. Respond to consumer’s needs in a timely fashion. Complete required documentation within established timeframes. Instill hope for the future by communicating and behaving with confidence and reassurance. Responsible for upholding a culture of privacy and security in a highly confidential work environment (HIPAA covered entity service area). Qualifications Associate’s Degree in Nursing (ADN) or higher Registration as an RN with WA Department of Health. Familiarity and experience with administering long-acting injectable buprenorphine in an outpatient setting, or strong interest in training for this specialized skill. Pass background check through Washington State Patrol. This position is a full time, M-F, first shift position, although flexibility in schedule and hours may be required for program needs 1 year experience working with vulnerable populations (either people living with homelessness, mental illness, or substance use disorders). Ability to outreach and engage individuals who are ambivalent about health care and treatment Core knowledge or ability to quickly learn Electronic Medical Records, tracking systems, and Excel spreadsheets. Excellent bedside manner and communication skills. Ability to take initiative in role development and act as a role model on care coordination and health promotion to other staff in the agency. Ability to explain treatments and conditions to adults of varying processing abilities. Active Driver’s license with reliable transportation and ability to drive between a variety of work locations throughout the day/weekly schedule. Preferred: Bachelor’s Degree in Nursing or BSN Equivalent Working knowledge of health care laws and regulations in Washington State and King County.
