Psychiatric RN Full time 40 hours Professional Elmwood Park, NJ, US Salary Range: $80,000.00 To $90,000.00 Annually Sign-on Bonus: $5,000.00 Location: Elmwood Park, NJ Work hours: Monday-Friday 8:00am-4:30pm Position Overview As part of the core services team, the PACT RN provides nursing assessment, direct‑support services and primary health care to seriously and persistently mentally ill individuals enrolled in the PACT Program. Provides support and education to other team members in nursing/medical treatment and health care. Essential Job Functions Frequent monitoring and assessment of the mental and physical health status of people served. Development of nursing assessment, comprehensive assessment and individualized recovery plan, providing knowledge and expertise from the nursing discipline. Population health component – prioritizing medical needs, tracking medical outcomes, facilitating follow‑up for medical conditions, and recommending age and gender appropriate screening (PSA, mammograms, colonoscopy, etc.). Participation in clinical and non‑clinical team meetings, including individual and team supervision. Actively seeks monthly supervision from supervisor and biannual group supervision with the Bridgeway medical APN. Provides, Teaches and/or Monitors Teaching of skills to support each person’s individualized recovery journey and community integration. Coordination of services with community mental health, non‑mental health providers and other medical professionals. Mental and physical health education, community living skill training and supportive counseling in the home and other natural environments. Assessment of service recipients’ stated concerns, mental illness symptoms, and behavior in response to medication; monitoring for medication side‑effects during observed self‑administration and ongoing contact. Medication education to individuals and family/significant others with service recipient consent. 24‑hour on‑call coverage on a rotating schedule, with the ability to respond immediately by phone or in person. Referral to a medical doctor for physical examination to be performed in the first 3 months of treatment. Rapid and flexible response to crisis, including accompaniment to local screening center or psychiatric emergency service and remaining with the person during assessment. Education of the individual regarding his/her illness and effects and side‑effects of prescribed medications. Symptom management efforts to help each consumer identify patterns and develop methods to lessen effects. Recognition of high‑risk medical and psychiatric conditions and appropriate action. Technical assistance and education to team members in psychiatric nursing and physical health. Metabolic syndrome screening – obtaining HgbA1c, fasting glucose, lipid profile, blood pressure. Administration of prescribed medications and injections, drawing blood, and preparing laboratory specimens for delivery, if certified. Administration of medications to enrolled individuals; training team members regarding medication education, delivery observation, and monitoring; assessing competency. Accurate record‑keeping of all medications administered in the medication record. Adherence to nursing and agency protocols regarding medication administration and storage; consultation with other service providers regarding assessment, treatment and rehabilitation needs. Assessment of individuals’ daily living skills and teaching budgeting, cooking, laundry, cleaning, shopping, personal hygiene, community transportation, self‑advocacy and housing maintenance. Use of Illness Management and Recovery and evidence‑based practices such as motivational interviewing, cognitive behavioral interventions and teaching strategies. Support to assist persons served to find and maintain employment and/or education/training. Timely and quality documentation adhering to DMHAS/Medicaid standards and inclusion of family and/or significant others as permitted. Support to family and social network to manage symptoms and reduce stress associated with illness; education to promote positive supportive interactions. Requirements Bachelor’s degree in nursing preferred. Valid NJ Registered Nurse license. One‑year experience working with individuals with serious and persistent mental illness. Valid driver’s license required. No more than one moving violation within the past 12 months. Vehicle required.
