Minimum Requirements

• Board Certified/Board Eligible: BC/BE in Emergency Medicine, Internal Medicine, or Family Medicine
• Licensure: Active, unrestricted Ohio medical license; Active DEA with Ohio address and schedules 2, 2N, 3, 3N, 4, and 5
