Roles & Responsibilities
- Prioritize and complete start of care visits for home health patients
- Supervise and support Physical Therapy Assistants to ensure high-quality care
- Complete clinical documentation within 48 hours of each visit
- Maintain timely communication with supervisors, intake, and office teams
- Coordinate care across a patient caseload in the Greater Sacramento area
- Be responsive and available during normal business hours to address patient needs
Qualifications
- Active and current California Physical Therapist license
- Minimum 1 year of Home Health experience
- Ability to supervise Physical Therapy Assistants
- Experience prioritizing start of care visits
- Must be responsive during business hours and meet documentation deadlines
Knowledge & Skills
- Strong clinical documentation skills, including Kinnser EMR
- Excellent communication and interpersonal abilities
- Supervision and delegation skills for therapy assistants
- Time management and prioritization in a home health setting
- Knowledge of start of care, recertification, evaluation, and discharge visit types
