• Schedule: Monday through Friday, 8:00 AM to 4:30 PM, with on-call rotation
• Shift Duration: 8 hours
• Hours: 40 hours per week
• Estimated Weekly Pay: $2,120 - $2,230
• Productivity Expectation: 30 points per week

Job Requirements

• Active Registered Nurse (RN) license
• Associate degree in Nursing or higher
• At least 1 year of nursing experience, preferably in home health nursing
• OASIS documentation experience required
• Strong clinical assessment, documentation, and communication skills
• Comfortable working independently and participating in an on-call rotation
• Ability to provide care in a home-based setting across the Denver metro area

Responsibilities

• Provide skilled nursing care to patients in their homes
• Perform wound care and support ongoing treatment plans
• Manage RN case coordination and complete start-of-care OASIS assessments
• Complete lab-related nursing tasks and monitor patient status
• Administer and manage medications safely and accurately
• Support catheter management and related patient needs
• Educate patients and families to promote comfort, safety, and independence
• Document care thoroughly and collaborate with the interdisciplinary healthcare team
