Deliver evidence-based physical therapy interventions to improve mobility, strength, balance, and functional independence. Monitor patient progress, document outcomes, and adjust treatment plans in collaboration with the interdisciplinary team. Educate patients and families on home programs, safety, and fall-prevention strategies; ensure patient safety during therapy sessions. Maintain accurate, timely documentation in the medical record and adhere to all facility policies and regulatory requirements. Requirements Active physical therapy license in good standing; NH licensure or eligibility may be required per regulatory standards. Doctor of Physical Therapy (DPT) degree or equivalent recognized by state licensing authorities. Basic life support (BLS) certification or per facility policy. Experience in skilled nursing facility or geriatric rehab settings preferred; strong communication and documentation skills. Ability to work effectively within a multidisciplinary team and adapt to a fast-paced care environment. Willingness to travel and commit to a 13-week assignment; able to meet credentialing and onboarding requirements.
