Responsibilities
- Conduct comprehensive PT evaluations and develop individualized treatment plans for adults in a skilled nursing setting
- Implement and document rehabilitation interventions to improve mobility, balance, strength, gait, and functional independence
- Monitor patient progress, adjust treatment plans, and communicate outcomes with the interdisciplinary team
- Educate patients and families on home exercise programs, safety, and strategies to optimize daily function
- Maintain accurate documentation in the electronic medical record and ensure compliance with facility policies and regulations

Requirements
- Active Physical Therapist license to practice in New Hampshire
- Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program
- Previous experience or demonstrated competency in geriatric rehabilitation or skilled nursing facility care
- Ability to work the stated day shift and meet guaranteed hours
- Strong communication skills and ability to collaborate with a multidisciplinary team
- Proof of eligibility to work in the United States and ability to travel for this assignment
