Position Overview As a Behavioral Health Case Manager, you will provide critical behavioral health support to children, adolescents, or adults experiencing emotional, behavioral, or developmental challenges. This dynamic role focuses on assessment, treatment planning, skill development, crisis prevention, and collaboration with interdisciplinary teams to improve client functioning across home, school, community, and clinical settings.

Key Responsibilities

• Assessment & Treatment Planning: Conduct comprehensive client assessments and develop individualized, goal-oriented treatment plans tailored to address mental health and substance abuse needs.

• Skill Development: Work directly with clients to teach and reinforce essential life skills, coping mechanisms, and emotional regulation strategies.

• Crisis Intervention: Proactively identify potential crisis triggers and implement effective crisis prevention and de-escalation strategies.

• Care Coordination: Collaborate closely with interdisciplinary teams—including therapists, medical staff, educators, and family members—to ensure a unified approach to client care.

• Community Integration: Facilitate services across various environments (home, school, clinical, and community) to bridge care gaps and support overall client functioning.

• Documentation: Maintain accurate, timely, and compliant clinical documentation and progress notes in accordance with state and agency standards.

Qualifications

• Bi-lingual preferred (Spanish)

• Active Ohio credentials or licensure preferred (e.g., QMHS, CDCA, LSW, LPC, or equivalent).

• Prior experience working with individuals facing mental health, behavioral, developmental, or substance abuse challenges.

• Strong understanding of trauma-informed care and dual-diagnosis treatment.

• Excellent communication, active listening, and problem-solving skills.

• Valid Ohio driver’s license and reliable transportation (as this role involves community-based work).

• Ability to pass a comprehensive background check.
