Roles & Responsibilities

• Provide excellent patient care by collaborating with the healthcare team to implement and revise case management programs.
• Act as a liaison between patients, families, nursing staff, and interdisciplinary teams to optimize patient outcomes.
• Coordinate care across the continuum for a defined patient population, from pre-admission to post-discharge.
• Ensure that patients and families navigate efficiently through the hospitalization process.
