Trinity Health PACE in Kinder Park is seeking a Primary Care Physician (MD or DO) to provide comprehensive primary medical care to participants, assess needs, and develop and implement a care plan in collaboration with the interdisciplinary team. You will collaborate with the Medical Director and IDT for referrals, follow up on outcomes, document care, and participate in ongoing quality assurance and staff training. #J-18808-Ljbffr IHA
