Registered Nurse – Care Facility
Our organization is looking for a dedicated and empathetic Registered Nurse to provide resident-focused care within our 24-hour nursing environment at our skilled nursing and rehabilitation center. The RN will be responsible for conducting assessments, creating and updating personalized care plans, administering medications and treatments, documenting in the electronic health record (EHR), and working collaboratively with a multidisciplinary team to assist both short-term rehabilitation and long-term care residents.
In this role, you will enhance dignity, safety, and quality of life for our residents through effective communication, education, and advocacy. You will also play a vital part in fostering a culture of respect, teamwork, and ongoing quality improvement.
Key Responsibilities:- Conduct thorough assessments of residents' health and needs.
- Develop and regularly update individualized care plans.
- Administer prescribed medications and treatments accurately.
- Maintain detailed documentation in the electronic health record system.
- Collaborate with an interdisciplinary team to ensure comprehensive care.
- Advocate for residents’ dignity and quality of life.
- Current Registered Nurse license.
- Experience in a nursing or rehabilitation setting is preferred.
- Strong communication and interpersonal skills.
- Ability to work effectively within a team.
- Commitment to continuous quality improvement and resident advocacy.
This position is based in a skilled nursing and rehabilitation facility with a focus on providing 24-hour care to residents.
