Requirements
- Bachelor's degree required
- Registered Nurse (RN) licensure, Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) required
- 3+ years of experience in overall acute care hospital case management required
- 2+ years of experience in case management leadership preferred

Roles & Responsibilities
- Direct and evaluate departmental operations, including case management model and staffing.
- Ensure adherence to care coordination and discharge planning processes.
- Assess and improve department performance through data analysis and implementing performance improvement activities.
- Establish working relationships with key stakeholders, including CMO, CNO, and ancillary service leaders.
- Manage departmental resources to achieve productivity and quality goals.
- Oversee compliance with policies and Standard Operating Procedures (SOP).
