ED Social Worker, LCSW — Crisis & Discharge Expert
Northwestern Medicine
The Northwestern Medicine Emergency Department seeks an experienced Social Worker (LCSW) to deliver psychosocial assessments, crisis interventions and discharge planning across patient populations on the Northwestern Memorial Hospital campus. This role collaborates with Patient Care staff, Medical Staff and ancillary departments to facilitate care, reduce hospital admissions and connect patients with community resources. Requires MSW and Illinois LCSW licensure; 24/7 coverage with 12-hour shifts. #J-18808-Ljbffr Northwestern Medicine
