Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to inform the development and implementation of tailored treatment plans and discharge planning.

• Collaborate effectively with an interdisciplinary healthcare team, sharing observations and insights regarding patient care while ensuring that treatment objectives are met.

• Engage in crisis intervention to support patients experiencing acute emotional or behavioral challenges, utilizing appropriate assessment tools and intervention strategies.

• Maintain clear and proactive communication with physicians and other healthcare providers to report patient progress, needs, and concerns, facilitating a cohesive approach to treatment.

• Generate and uphold accurate documentation, including clinical notes and progress reports, adhering to departmental policies to ensure compliance and optimize the quality of care.

Patient Population / Client Focus\n

You will work with a diverse range of clients, including children, adolescents, and adults, each facing unique challenges that require tailored interventions. Qualifications\n

• Master’s degree in Social Work or related field, with active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.

• Proficient in crisis intervention strategies and familiar with a variety of therapeutic modalities that are effective in outpatient treatment.
