Key Responsibilities\n

• Conduct comprehensive assessments and create individualized treatment plans that address patient needs while considering pre-crisis and crisis scenarios.

• Deliver timely consultation and intervention in behavioral guidance, risk assessments, and medication treatment plans.

• Utilize evidence-based assessment tools in collaboration with the clinical team for better patient outcomes.

• Document all patient interactions and treatment progress according to regulatory standards to ensure compliance and quality of care.

• Work closely with interdisciplinary teams to design and implement effective positive behavior support plans tailored to individual patients.

• Engage in continuous advocacy for patients by liaising with relevant service systems, community partners, and state agencies to align resources with their needs.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker at Retreat Behavioral Counseling Center, you will primarily serve a diverse patient population coping with serious health challenges, including terminal illnesses. Your expertise will help patients navigate complex emotional and psychological issues as they receive care within a hospice setting. Your role will also extend to supporting families in contentious or stressful situations, offering guidance and advocacy to ensure they receive the best possible support.

Qualifications\n

• A Master’s Degree in Social Work (MSW) from an accredited educational institution.

• A current state license as a Clinical Social Worker (LCSW) in good standing.
