Your Role as an LPN:

As a Licensed Practical Nurse , you'll lead your care team in providing total, resident-centered care. Working closely with Nurse Managers and interdisciplinary teams, you’ll ensure that each resident’s care plan is followed with compassion, professionalism, and heart .

Key Responsibilities:

• Take responsibility for total care of residents on your assigned team

• Administer medications and treatments as prescribed

• Collaborate with residents, families, and other care providers

• Promote independence, dignity, and a meaningful quality of life

• Serve as a mentor and role model to other care staff

• Respond to emergencies and exercise strong clinical judgment

What You Bring:

• Current LPN license (Pennsylvania)

• Rehab or long-term care experience preferred

• Strong communication & leadership skills

• Flexibility with scheduling

• A caring, resident-first mindset and commitment to team success

• Ability to work collaboratively with physicians and interdisciplinary staff

• Computer proficiency preferred (EMR experience a plus)
