Registered Nurse (RN) Case ManagerAre you an experienced Registered Nurse looking for a role that offers meaningful patient relationships, professional independence, and flexibility outside of a traditional bedside setting?Right at Home Boston Northwest is growing, and we're looking for a compassionate and organized Registered Nurse (RN) Case Manager to join our home care team.This is a field-based home care position serving clients throughout our Boston Northwest service area, including Bedford, Lexington, Burlington, Woburn and surrounding communities. Local travel between client homes is required.Working alongside our Director of Nursing and care team, you'll help oversee the care of older adults who want to remain safe and independent in their homes. This is an excellent opportunity for an RN who enjoys clinical assessment, care coordination, working directly with families and caregivers, and having more independence than a traditional bedside role.What You'll DoConduct in-home nursing assessments and reassessments for new and existing clientsDevelop, review, and update individualized plans of careProvide ongoing clinical case management and follow-up visitsMonitor changes in client condition and coordinate appropriate clinical follow-upEducate, coach, and support caregivers in delivering safe, high-quality careServe as a clinical resource for clients, families, caregivers, and our office teamCollaborate with the Director of Nursing, scheduling, and care management teams to maintain continuity of careCommunicate with physicians and other healthcare professionals when appropriateSupport clients and families during changes in care needs and transitionsMaintain accurate and timely clinical documentationBuild positive relationships with clients, families, caregivers, and community healthcare partnersSchedule & FlexibilityWe are open to both full-time and part-time candidates. Our preference is for an RN interested in 32–40 hours per week, but we don't want to miss a great nurse simply because they need a reduced schedule. Qualified candidates seeking approximately 24 or more hours per week are encouraged to apply.This is a mobile, field-based role involving local travel to clients' homes throughout Burlington and our broader Boston Northwest service area.Compensation & Benefits$45–$52 per hour, based on experiencePaid mileage for work-related travelPaid Time Off (PTO)Health insuranceDental insuranceVision insurance401(k) with up to a 4% employer match(Benefit eligibility may vary based on scheduled hours and employment status.)What We're Looking ForActive, unrestricted Registered Nurse (RN) license in Massachusetts2+ years of nursing experienceHome care, geriatrics, private-duty nursing, or case-management experience preferred, but not requiredStrong clinical assessment and judgment skillsExcellent communication and interpersonal skillsStrong organization, time-management, and documentation skillsComfortable working independently while remaining part of a collaborative clinical teamCompassionate and dependable, with an interest in working with older adults and their familiesValid driver's license and insured vehicleAbility and willingness to travel locally between clients' homesWhy Right at Home Boston Northwest?This isn't a traditional bedside nursing position.
