Health Care Coordinator (Nurse)

This position is responsible for the provision of Wellness Coordination Services to manage health care needs of Intellectually Disabled / Developmentally Disabled individuals. The position requires attention to detail, accurate, thorough and timely documentation, a commitment to individual services and a team mentality. Reports To: Executive Director (as to programmatic issues) President (as to nursing) Experience and Qualifications: At least 25 years of age High School Diploma or GED equivalency CPR certification Free from communicable diseases Owns or has access to a vehicle in good working order Current automobile insurance (for vehicles registered in the state of Indiana) Clean criminal history and OIG record No findings on any aide registry Successful completion of Training and Orientation program Competency/Skill Requirements Strong Assessment and Observation Strong Reporting Skills Ability to document in a timely and accurate manner according to principles of documentation Excellent interpersonal and communication skills Ability to accomplish tasks with minimum supervision in a timely manner Strong observation, organization and problem solving skills #J-18808-Ljbffr Residential Services Incorporated
