Life Coach & Case Manager — Housing-First Support
Bond Health
Bond Health is seeking a Life Coach to provide case management and service coordination at Melrose, a 78-unit Permanent Supportive Housing program in the Bronx. You will support adults with mental illness and substance use histories, using Housing First, Harm Reduction, and trauma-informed approaches, and work with a multidisciplinary team to link residents to benefits and community resources.
