Behavioral Health Provider - CSS

Role Overview As a Behavioral Health Provider (BHP) in the Health Department, you will be part of the primary‑care treatment team that identifies, triages, and manages patients with medical and behavioral health needs within the primary care setting. The BHP provides brief, solution‑focused behavioral interventions, psycho‑education, and behavioral change plans to patients referred by medical providers. The position requires coordination in crisis situations, stabilization of patients, and ongoing monitoring of patient behavior. The BHP must be a licensed professional in Oregon—either a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC). Key Responsibilities Deliver brief (10‑20 minute) behavioral interventions and behavioral change planning for patients referred by medical providers. Provide follow‑up psychotherapy related to mind‑body connections with medical problem presentations. Conduct biopsychosocial assessments and apply evidenced‑based psychotherapy interventions to patients across the lifespan with medical conditions. Document assessments, progress notes, and billing for funding sources. Be immediately available during working hours for psychiatric or behavioral health emergencies, leading stabilization and follow‑up. Serve as a behavioral health consultant to Primary Care Provider (PCP) teams, offering education on psychosocial treatments. Partner with other providers, staff, and family or support systems to develop and maintain care plans. Conduct individual, couples, or family consultation, including functional assessments, psycho‑education, medication adherence counseling, motivational interviewing, and brief cognitive interventions. Provide case‑management services and coordinate care with community behavioral health partners. Advocate for and refer patients to specialty behavioral health services when needed. Required Qualifications Licensed Clinical Social Worker (Oregon Board of Licensed Social Workers) or Licensed Professional Counselor (Oregon Board of Professional Counselors). Minimum of 1 year of experience working with adults diagnosed with HIV/AIDS. Preferred Qualifications Experience effectively responding to crisis situations. Understanding of medical systems and complex medical/mental health issues. Knowledge of the local behavioral health treatment system and community resources.
