Case Manager

Coleman Health Services, a nationally recognized not-for-profit mental health and substance use services provider is seeking a full time Case Manager to provide case management services to individuals with severe and persistent mental health issues who may also have a substance abuse disorder in Logan and Champaign counties. The Case Manager will assist with accessing community‑based resources, housing, employment and linking the individual with ongoing mental health and/or substance abuse services. The Case Manager will work with Probation, Parole, Courts, Social Security, DJFS and other providers. Must have BA in related field, one-year related experience and knowledge of community resources. Transition clients and families into ongoing care Work collaboratively with treatment teams internally and externally to ensure safe transitions Qualifications of the Case Manager Bachelor's degree in a social service‑related field or three‑years' experience Willing to get NPI and Medicaid Billing number (we\'ll help!)
