Responsibilities
- Conduct initial PT evaluations for SNF residents and develop individualized treatment plans.
- Deliver and adjust therapy interventions to improve mobility, balance, strength, and activities of daily living.
- Monitor patient progress, document outcomes, and communicate changes to the interdisciplinary team.
- Ensure accurate and timely documentation in the patient record and maintain compliance with regulatory standards.
- Support discharge planning and patient/caregiver education to promote continued rehabilitation goals.

Requirements
- Active Physical Therapist license in New Hampshire or eligibility for licensure.
- DPT or equivalent degree from an accredited program (state licensure required to practice).
- Ability to work a 40-hour week in a fast-paced skilled nursing facility environment.
- Experience in geriatric rehabilitation or SNF settings preferred.
- Strong communication and collaboration skills; commitment to patient safety and quality of care.
