Responsibilities Assess patients’ functional status and home environment for hazards or barriers, identify equipment needs, and participate in development of total plan of care. Execute treatment program and goals for improved function and communicate plan of care to referring physician and other organization professionals. Teach new skills or retrain patients in daily activities that have been lost due to illness or injury. Fabricate splints and instruct patients in use of adaptive equipment. Train patients in use of prosthetic and/or orthotic devices. Maintain appropriate clinical records, notes, and summary reports, and report to physician any changes in patient condition in accordance with policy. Maintain contact and communication with other personnel involved in patient care to promote coordinated, efficient care. Attend and contribute to in‑services, case conferences, and other meetings as required. Identify patient and family/caregiver needs for other home health services and refer as appropriate. Instruct patient’s family/caregiver and other health care personnel in patient’s treatment regime when indicated. Participate in discharge planning for patient. Qualifications Licensed or otherwise regulated, if applicable, as an occupational therapist assistant by the state in which practicing. Graduate after successful completion of an occupational therapy assistant education program accredited by ACOTE. Demonstrates good verbal and written communication and organization skills.
