Care Manager - Behavioral Health

About This Role The Care Manager - Behavioral Health develops, assesses, and facilitates complex care management activities primarily for members with mental and behavioral health needs. This role is ideal for individuals passionate about providing high-quality, cost-effective healthcare outcomes. Highlights Pay: $56,200 - $101,000/year Location: Remote, MO Schedule: Full Time, Day shift What You'll Do Evaluate the needs of the member via phone or in-home visits and recommend/facilitate care plans. Perform telephonic, digital, home, and/or other site visits outreach to assess member needs. Develop ongoing care plans for members with high acuity and identify necessary providers and resources. Coordinate between the member, family/caregivers, community resources, and care provider team. Monitor care plans/service plans and member status for changes in treatment and provide recommendations. Facilitate care coordination and collaborate with providers or specialists for timely access to care. Collect, document, and maintain member information and care management activities for compliance. Provide education to members and their families on procedures, treatment options, and healthcare benefits. Provide feedback to leadership on opportunities to improve care and quality delivery. Perform other duties as assigned and comply with all policies and standards.
