LPN Position AvailableCountryside Health Care of Milford currently has LPN positions available on the 3-11 & 11-7 shifts. We are looking for dynamic, team oriented LPN's who enjoy working with a variety of patients to assist them with obtaining optimum function either within the LTC setting or to assist them with their successful return to home from our active Rehabilitation Unit.Purpose: To conduct and coordinate all nursing activities on the assigned unit in a safe, timely and accurate manner in accordance with state and federal requirements and the policies and goals of this facility.Duties and Responsibilities:Receive and give accurate and concise report on each resident/patient at change of shiftGive report to C.N.A.'s at beginning of shift and receive from them before end of shiftOversight and management of C.N.A.'s assigned to that unit, including daily assignments, setting and monitoring breaks and evaluating needs for improvementAuthority to discipline/counsel C.N.A.'sManagement of all resident/patient care on that unit, including updating nursing care plans and C.N.A. care plans with changes and new ordersReview care plans for appropriate problems, approaches, goals and revisionsReview C.N.A. documentation, vital signs, bathing, weights, I&O's, BM'sComplete nursing summaries and quarterly assessments as assignedMake daily rounds and is aware of all resident/patient conditions on that unit and reports pertinent data to Unit Manager or designeeNotify MD/NP and resident/patient responsible party of significant changes in conditionEvaluate resident/patient needs and the effectiveness of care providedPromote effective restorative nursing techniquesMaintain universal precautions and infection control policiesDocument completely and accurately using only standard abbreviationsAdminister and record medications as ordered and maintain accurate narcotic count according to pharmacy and facility policyMaintain/order/obtain adequate medications, nursing supplies and equipment as necessary for the unitAssist with editing and end-of-month changeover as assignedWeekly skin checks and administration of treatments as ordered, documenting changes and progressFinger stick blood sugars as ordered with accurate documentation and notification of MD/NP if outside parametersObtain specimens, follow up with lab and notify MD/NP of resultsAssist with meal trays, call lights and alarmsClean, re-stock and empty trash on medication and treatment carts before change of shiftContact MD/NP/families as necessaryTranscribe new orders promptly and accuratelyAdmit, transfer and discharge residents on assigned unit and notify business office and dietary of changesComplete incident reports and document in the medical record according to facility policyComplete nursing log at end of shiftAssist with orientation and staff developmentParticipate in staff meetings and continuing education opportunitiesDeliver direct resident/patient care when necessaryAssume supervisory responsibilities if requested by the DNS or designeePerform other duties as assignedBenefits:403BDental insuranceDisability insuranceHealth insuranceLife insurancePaid time offEmployee DiscountsSchedule:8 hour shiftDay, Evening, or Night ShiftHolidaysWeekendsFull-timeSalary: $29.00 - $45.00 per hour based on years of experience and differential of shift worked.Sign on Bonus!Bonus Structure$1,000 after 90 days of employment from date of hire$1,000 after 6 months of employment from date of hire$1,500 after 1 year of employment from date of hireRequirements:Nursing: 1 year (Preferred)LTC: 1 year (Preferred)Licensed Practical Nurse (LPN) (Required)Computer literacy is desirableExcellent communication and interpersonal skillsMust meet the general health requirements set forth by the policies of this facilityAble to push, pull, move and/or lift a minimum of 25 pounds for at least 50 feet with or without the aid of mechanical devices
