Case Manager

Join our client's team as a case manager and make a meaningful difference in patient care! Are you passionate about healthcare and dedicated to improving patient outcomes? We are seeking several compassionate and detail-oriented case managers to lead care coordination efforts for individuals with chronic illnesses. In this critical role, you will serve as a vital link between patients, healthcare providers, insurance companies, and support resources ensuring timely access to therapies and a positive journey through the healthcare continuum. If you thrive in a collaborative environment and want to contribute to impactful patient advocacy, this opportunity is for you.

Required skills:

  • Strong communication and interpersonal skills
  • Excellent case management and care coordination abilities
  • Experience in health care insurance benefits, laws, and regulations
  • Ability to assess ethical and legal aspects of patient care
  • Proficiency in data entry and computer skills
  • Capable of managing complex cases independently
  • Bilingual abilities (Spanish) a plus

Nice to have skills:

  • Background in medical, insurance, or customer service industries
  • Experience with healthcare reimbursement processes and appeals
  • Knowledge of billing, coding, and regional payer trends
  • Mediation and problem-solving expertise

Preferred education and experience:

  • Bachelor's degree in healthcare, social work, nursing, or related field
  • Minimum of three years' recent professional experience in health care insurance, patient advocacy, or case management
  • Prior experience in home care management, utilization review, or social services

  • Must pass background check
