Job Summary and Qualifications The Lead Social Worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated. Other Duties & Responsibilities: Acts as a liaison between the patient and community resources/post-acute care providers. Creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. Facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput. Provides orientation and mentorship to new and existing staff, collaborates with the Director of Case Management in the hiring and interview process for new professional staff and manages a limited case load in addition to providing guidance and consultation to social work staff. What qualifications you will need: A Master’s Degree in Social Work is required Licensure as a LMSW or a LCSW is required At least 2 years of experience as a Social Worker is required Prior experience in the acute care setting strongly preferred Benefits HCA Healthcare Trident Hospital offers a total rewards package that supports the health, life, career and retirement of our colleagues.
