PURPOSE:A Community Health Worker (CHW) serves as a trusted liaison between healthcare providers, social services, and the community. The CHW supports individuals and families by providing health education, connecting them with resources, promoting wellness, and helping reduce barriers to care.SUPERVISORY AUTHORITY:None.NATURE AND SCOPE:Community settings, clinics, nonprofit organizations, or public health departments. May include outreach events, and occasional evenings/weekends Responsible for advocacy and problem-solving, health education and communication, and collaboration and teamwork.PRINCIPAL ACCOUNTABILITIES:Provide culturally appropriate health education and outreachConnect clients with medical, behavioral health, and social service resourcesAssist individuals with care coordination and follow-up appointmentsConduct community outreachHelp clients navigate healthcare systems and insurance processesAdvocate for community needs and support health equity initiativesDocument client interactions and maintain accurate recordsParticipate in case conferences and interdisciplinary team meetingsSupport chronic disease managementBasic computer skills in operating systems, internet & email proficiency, Word, Excel, file management, create documents, organize files and solve simple problems related to the computer and smart phone technologyCORE COMPETENCIES:Knowledge of community resources and local populationsStrong communication and interpersonal skillsAbility to build trust with diverse communitiesBasic computer and documentation skillsMINIMUM REQUIREMENTS:High school diploma or GED (associate or bachelor’s degree in public health, social work, or related field preferred)Compliance Salary Range Disclosure$18.40-$20.00 per hour #J-18808-Ljbffr Hastings Center, Inc.
