Important things to know about the job:

Full-time position, 40 hours per week

JOB SUMMARY: The case manager is a RN or LPN who collaborates with interdisciplinary team members, patients, and families to facilitate the transition of the patient from the hospital to a post-hospital setting. The case manager plays an integral part in the utilization and resource management process.

QUALIFICATIONS:

• Licensed in the state of Oklahoma.
• Minimum of 2 years clinical experience.
• Case management experience preferred.
• Strong skills in oral and written communications.
• BSN preferred.

PHYSICAL REQUIREMENTS:

• Able to speak clearly and distinctly with staff, physicians, patients, and families.
• Able to extensively utilize telephone for communication.
• Demonstrates adequate physical and emotional stamina to deal with stressful situations and to complete the necessary work schedule.
• Able to sit/stand/walk for extended periods of time throughout the work schedule, with some light lifting and bending.
• Demonstrates adequate vision, or correctable with glasses/contacts, for reading and preparing written documents.
• Must have adequate perception of sounds or adequate hearing with corrections.
• Able to hear spoken communications from staff, physicians, patients, and families as well as pages.
• Demonstrates adequate manual dexterity to prepare written documents.
• Able to work in a latex burdened environment.
