Prototypes is one of the nation’s leading behavioral healthcare providers and is a “prototype” for such organizations. Prototypes Women's Center in Pomona is a 180-bed residential facility tailored specifically to women, including those who wish to involve their children in their journey to recovery. Our program aims to assist women in maintaining their progress in overcoming co-occurring disorders through a wide range of supportive services. Children can foster a secure attachment with their mothers in a secure setting, while also accessing mental health support and educational assistance. Our interdisciplinary team of professionals provides comprehensive treatment for substance use disorders and mental health, parenting, educational, and life skills courses. Treatment can range from 60 days to 6 months. The Case Manager I provides in-person support to clients by coordinating care both within HealthRIGHT 360 and with outside service providers. They work closely with clients to complete intake paperwork, conduct outcome assessments, and apply for benefits. As part of a multidisciplinary team, the Case Manager I helps ensure effective treatment that aligns with HealthRIGHT 360’s mission, values, and policies. Services are delivered flexibly in outpatient offices, residential facilities, or in the community, depending on client needs and program requirements. Therefore, reliable transportation is required. KEY RESPONSIBILITIES Direct Service Assist the client with intake by completing case management assessment and entering financial and benefit information. Support the client in applying for Medi-Cal or transferring Medi-Cal County when appropriate. Assess the client’s case management needs and complete all releases of information. Connect the client to benefits, mental health, physical health, employment, probation, DCFS, housing, community resources, outpatient substance use disorder services, and aftercare. Coordinate communication and external service linkage including assisting with scheduling appointments, communicating with probation, scheduling child visits, communicating with DCFS, obtaining all court minute orders, providing appointment reminders for therapy and psychiatrist. Deliver services in outpatient, residential, or field depending on assignments and client need. Complete the VI-SPDAT and connect the client to safe housing options. Prepare the client for job search by assisting with resume writing and job search preparation. Monitor client's progress toward achieving case management treatment plan goals and provide treatment plan input. Seek clinical consultation throughout the treatment episode and when encountering new and/or high-risk clinical circumstances. Work collaboratively with other members of the treatment team including the Clinical Therapist, Counselor, Medical, and Psychiatrist. Communicate collaboratively with all members of the behavioral health team including medical, mental health, psychiatry, substance use disorder, and other staff. Transport clients as needed. Complete client UA tests and document test results. Documentation Write and complete all progress notes within 72 hours of service delivery. Maintain compliance with the due dates for all clinical documentation. Write client’s progress letters and court reports and obtain final approval from the supervisor. Complete all documentation in compliance with agency policy and procedures, HIPAA, 42-CFR, DMH, and SAPC standards. Complete all assigned URC’s within a timely manner. Administration and Other Duties Assist with client program oversight by managing case management business passes. Meet expected client care hours (productivity expectation is 25 client care hours per week). Maintain full compliance with requirements for registration or certification. Attend and actively participate in assigned Clinical Group Supervision. Actively participate in agency and team meetings. Participate in training opportunities and complete all assigned training in a timely manner. Arrange work schedule in accordance with the agency’s needs. Read emails daily and respond when appropriate. And perform other duties as assigned. QUALIFICATIONS Education, Certification, Licensure, and Experience Option I: High school diploma or equivalent. Substance Use Disorder Counselor registration from an accredited California agency (CCAPP, CAADE, CADTP). Option III: Bachelor’s degree in Social Work, Psychology, or other related field from an accredited institution. Experience working with clients experiencing acute withdrawal from substances. Experience in the human service field and demonstrated expertise in substance abuse treatment, relapse prevention, and recovery. Valid First Aid and CPR certification or ability to obtain within 30 days of hire. Valid California Driver’s License and access to registered and insured transportation. Assessment Skills. Ability to conduct thorough case management assessments, including intake procedures and outcome measures. Care Coordination. Ability to navigate internal and external resources to support clients’ treatment plans. Ensure smooth transitions between services to maintain continuity of care. Communication Skills . Effective verbal and written communication skills to coordinate external service linkage, maintain client communication, and collaborate with various stakeholders. Community Knowledge . Familiarity with local community behavioral health and public health systems to make proper referrals. Cultural Sensitivity. Ability to interface well with diverse, disadvantaged populations including those with history of incarceration, substance use, or mental health issues while withholding judgment. Data Management . Ability to enter client records accurately and maintain client records in electronic databases. Field Readiness . Comfortable working in a variety of settings based on client and program needs. Integrity and Ethics . Adherence to confidentiality laws, ethical guidelines, and organizational policies, and demonstrate accountability, reliability, and integrity in all actions. Teamwork . Accountable to the team and positively collaborate with other team members and community-based providers. Technical Skills. Ability to use electronic health records (EHR) and other tools for case management. Background Check and Other Requirements Qualified candidates with a criminal record will be considered for employment. Must be capable of obtaining and maintaining a satisfactory background check. Must be capable of meeting health screening and tuberculosis testing requirements. Must be capable of meeting program and funding requirements. Must be capable of maintaining credential requirements, if applicable.
