Responsibilities Conducts nursing assessments; obtains, records, and updates individuals’ health information and vital signs using standard instruments and procedures. Prepares, implements, and modifies nursing care plans; assists with examinations and treatments; administers medications and nursing procedures as prescribed. Observes, monitors, and records individuals’ symptoms, conditions, and care, including diet and physical activity; identifies and reports responses to medication or changes in health status. Serves as the nursing representative on a treatment team; provides information on nursing needs and consults with other health care disciplines to assess, plan, implement, or evaluate care. Makes clinical rounds to assess nursing care needs and participates in the provision of nursing services. Provides health teaching, counseling, and education to individuals, families, and groups; develops health improvement programs. Implements infection control protocols and provides related consultation and instruction to staff. Maintains and updates charts, records, and other documentation in compliance with agency, state, and federal requirements; may collect data to support medical research. Monitors inventories and ensures standards for medications, solutions, supplies, and equipment meet nursing care needs. Clinical Supervision and Instruction Delegates nursing activities, skills, and procedures to nursing and direct care staff; makes clinical rounds to assess and evaluate nursing services. Acts as liaison to outside agencies and providers to ensure continuity of care and necessary follow‑up; advocates for individuals’ needs; may provide instruction on discharge plans. Performs supervisory duties over Graduate Nurses, Licensed Practical Nurses, and direct care staff; provides information to Registered Nurse Supervisors; prepares reports and performance evaluations for lower‑level staff involved in nursing care activities. Position Specific Duties Provides nursing services to a caseload of approximately 20 developmentally disabled individuals with medical or increased psychiatric needs. Provides RN oversight for approximately 40 unlicensed staff on tasks such as assessments, weekly site visits, nursing triage, clinical supervision, plan development, staff training, medication certification, error tracking, and medication audits. Minimum Qualifications Must hold a current registration as a registered professional nurse in New York State. Benefits and Compensation Salary: PS&T Grade 18 ($66,951) + Increased Hiring Salary ($9,196) + Geographic Pay Differential ($20,000) = $85,138. Paid holidays: 13 per year; paid vacation: 13 days initially; paid personal leave: 5 days per year; paid sick leave: 13 days per year (carryover possible); professional leave: up to 3 days per year for professional development.
