Licensed Practical Nurse - Long Term Care (CLC)

Job Summary The Western New York VA Medical Center is seeking highly qualified Licensed Practical Nurses to serve Veterans in our Community Living Centers (Long Term Care) as valuable members of the Geriatric and Extended Care Team. The position is located in scenic Batavia, New York. Recruitment incentive of $7,500 is authorized for highly qualified candidates. Duties Work under the direction and supervision of a registered nurse, providing a full range of practical nursing care to patients with physical and/or behavioral problems. Serve as a responsible member of the nursing team, interacting with patients, families, and professional and supportive personnel. Participate as a member of the interdisciplinary team, providing holistic care in acute, sub‑acute or chronic stages of illness. Establish positive patient/family/staff relationships using appropriate communication skills. Assist in the collection and documentation of routine objective and subjective data for uncomplicated patient situations. Maintain a clean, safe, and therapeutic environment and adhere to all safety and health standards. Provide competent patient care ensuring safety to patients, staff, visitors, and self, and seek direction as needed. Assist with simple diagnostic measures, including collection of urine and stool specimens, and other specimens as instructed. Respond to emergency situations; initiate CPR when indicated, maintaining BLS certification. Perform routine treatments relevant to area of assignment. Administer selected medications, seeking clarification of orders as necessary; identify and document PRN medication needs and effects. Document medication administration, PRN response, and basic information in the medical record, reporting patient status to RN. Document teaching provided to patients/families and the response. Maintain respect for human dignity and protect patient privacy. Assist with the implementation of the unit‑level quality improvement program. Maintain professional boundaries with patients, families, and staff at all times. Work full‑time with a 12‑hour tour of duty (days/evenings/nights). Tour preference to be discussed at interview. Education and Experience Only education or degrees recognized by the U.S. Department of Education from accredited institutions qualify for federal employment. Qualifications U.S. citizen. Proficient in written and spoken English. Must pass pre‑employment physical evaluation. Driver’s license or state‑issued ID must be REAL‑ID compliant effective May 7 2025. Background/security investigation required. May be required to complete drug screening; refusal denies employment. Participation in the seasonal influenza vaccination program required. Must serve a 1–2 year trial period subject to performance review.
