RN Case Manager, Per Diem

About This Role The RN Case Manager, Per Diem at Hackensack Meridian Health is responsible for coordinating patient care activities to ensure compliance with evidence-based practice standards and regulatory requirements. Highlights Location: Hackensack, NJ Schedule: Per Diem, Rotating What You'll Do Coordinate patient care progression throughout the continuum. Facilitate collaborative management of patient care, intervening to remove barriers. Apply process improvement methodologies to evaluate care outcomes. Communicate with physicians, nursing, and ancillary staff. Evaluate care against Length of Stay metrics. Collaborate with Admitting for accurate patient demographic and insurance information. Communicate medical plan of care and discharge plan to patients and families. Assess and plan for discharge needs and arrangements. Complete utilization management for assigned patients. Monitor length of stay and clinical resource use. Requirements Education Bachelor's degree in nursing (BSN) preferred; 1 year experience for non-BSN HackensackUMC employees. Experience 2+ years clinical experience in relevant specialty preferred. Minimum of 2 years in Case Management in an acute care setting preferred. Licenses & Certifications Current NJ State Professional Registered Nurse License required. BLS AHA Basic Health Care Life Support HCP Certification required.
