Travel MDS Coordinator RN

Champion Care is seeking an experienced Travel MDS Coordinator RN to support our skilled nursing and rehabilitation communities throughout Wisconsin. This is an exciting opportunity for an MDS professional who enjoys variety, autonomy, and the opportunity to make a meaningful impact across multiple facilities.

Instead of being limited to one building, you will serve as a trusted clinical and regulatory resource for our nursing teams throughout Wisconsin. You will help strengthen MDS accuracy, care planning, compliance, and reimbursement while working alongside facility and regional leadership.

What You Will Do

• Complete and coordinate accurate, timely Minimum Data Set assessments
• Support the Resident Assessment Instrument process in accordance with CMS, OBRA, and state requirements
• Review clinical documentation for accuracy, completeness, and reimbursement opportunities
• Coordinate interdisciplinary care planning and ensure resident needs are accurately reflected
• Monitor assessment schedules, required documentation, and submission deadlines
• Provide education and support to MDS Coordinators, nursing leaders, and interdisciplinary teams
• Assist facilities with PDPM, case mix, quality measures, and survey readiness
• Identify opportunities to strengthen documentation, processes, and resident outcomes
• Serve as an MDS resource during vacancies, transitions, and periods of increased need
• Travel regularly to Champion Care communities throughout Wisconsin

What We Are Looking For

• Current Registered Nurse license in Wisconsin or eligibility to obtain one
• Previous MDS Coordinator experience in skilled nursing or long-term care
• Strong working knowledge of MDS 3.0, RAI guidelines, PDPM, OBRA assessments, care planning, and CMS requirements
• Ability to manage assessment schedules and meet required deadlines
• Strong clinical judgment, organization, and attention to detail
• Confidence working independently and supporting multiple nursing teams
• Excellent communication, education, and problem-solving skills
• RAC-CT certification is preferred but not required
• Willingness and ability to travel throughout Wisconsin
