Key Responsibilities\n

• Act as a liaison for clients, families, and outpatient providers to ensure comprehensive and integrated care.

• Engage with insurance companies to facilitate pre-certification processes and manage utilization reviews for services.

• Participate in collaborative discharge planning and aftercare services to support client transitions.

• Provide therapeutic meal support that encourages healthy relationships with food, tailored to the individual needs of clients.

• Deliver safe, empathetic, and affirming support, recognizing and respecting the intersecting identities of each client.

Patient Population / Client Focus\n

Qualifications\n

Compensation and Benefits\n

The salary range for this role is competitive, falling between $90,000 and $105,000 annually, commensurate with experience and qualifications.
