LPN Med Care Manager

Role: Licensed Practical Nurse (LPN) Medication Nurse in a senior care setting. Responsibilities: Administer medications and treatments following physician orders, adhering to the Six Rights of Medication Administration. Maintain accurate clinical documentation in MAR/TAR, monitor residents' symptoms, report adverse reactions or refusals promptly, and support care plan updates. Conduct Service & Health Updates (SHUs), transcribe physician orders, observe and report physical, emotional, or cognitive changes, and ensure medication carts are maintained with proper narcotic counts. Play a vital role in preventing medication errors, early detection of health changes, and safeguarding resident safety through clinical precision and regulatory compliance. Qualifications and Skills: Active LPN license in good standing, CPR and First Aid certification. Strong assessment and documentation skills are essential. Experience in senior care or medication administration is preferred.
