This leadership role provides the opportunity to directly influence patient outcomes, discharge planning success, utilization management, and team performance while partnering closely with executive leadership. Competitive salary of $90,000-$105,000 plus quarterly bonus eligibility Direct reporting relationship to the hospital CEO Support from a Regional Director of Case Management Opportunity to lead and develop an established Case Management team Relocation assistance may be available Day one eligibility for medical, dental, vision, PTO, holiday pay, 401(k), and additional benefits Stock option eligibility for directors and above Stable hospital environment with strong patient census Certification support for CCM or ACM if not already obtained Job Details Direct all Case Management department operations for a 50-bed inpatient rehabilitation hospital Lead, mentor, and develop a team of Case Managers and Case Management support staff Oversee utilization review, discharge planning, care coordination, and patient progression activities Ensure compliance with hospital policies, regulatory standards, and best practices Develop strategies to maximize quality outcomes and efficient patient transitions of care Monitor departmental performance metrics and implement process improvements Collaborate closely with physicians, nursing leadership, therapists, and interdisciplinary teams Support case management staff during high-volume periods and assist with patient coverage when needed Foster a positive team culture focused on accountability, patient experience, and operational excellence Work directly with executive leadership to achieve organizational goals and performance objectives Support certification and professional development initiatives within the department Jobot is an Equal Opportunity Employer.
