Staffing: 3 APPs plus nursing staff Supervision: None required Patient Population Clinical Responsibilities The provider will evaluate treat and monitor patients with the following conditions: Migraines Stroke Seizures Multiple sclerosis MS Sleep disorders Restless leg syndrome Procedures: No procedures required No EMGs No sleep studies Provider Requirements: Board Certification: Required State License: Active New Hampshire NH license required BLS: Required DEA: Required Minimum Experience: 1 year Willing to License: Yes but currently licensed candidates are preferred Name Clear: Provider name and NPI required Rates Billing: Overtime: Same rate as regular hours Orientation: 8 hours non-billable Holiday Rate: 1.4x regular rate Travel Expenses: Open to flights hotel and rental car.
