Coordinator HCBS - FT

RISE is an innovative human services network that has supported children, adults, and families with a variety of needs since 1987. RISE has an opening for a Service Coordinator to oversee our Home and Community Based Services (HCBS) and Managed Care (MC) programs. The Service Coordinator is a safety‑sensitive position and provides leadership and support to Direct Support Professionals and the clients and families we serve. The ideal candidate has experience working with individuals with intellectual and/or developmental disabilities and the elderly. Responsibilities Manage and support HCBS programs, implementing RISE mission, vision, and values and delivering hands‑on impact. Supervise Direct Support Professionals (DSP) staff. Establish partnerships with RISE customers, including clients, families/guardians and funding agencies. Oversee and manage program records. Prepare and maintain payroll and billing documentation accurately and within designated time frames. Ensure programs meet licensing/survey requirements and contractual compliance. Qualifications High School Diploma or GED and at least two years of supervisory experience. Fluent bilingual Spanish speaker in both verbal and written communication. Experience working with the elderly and/or physically disabled in a long‑term setting. Ability to work flexible hours and self‑directed with minimal supervision. Travel within the county where HCBS services are provided or interstate travel may be required. Additional Candidacy Requirements Must possess a valid Arizona driver’s license, be at least twenty‑one years of age, and meet driver requirements specified by RISE Insurance Agency. Must successfully complete a background check, driving record check, drug test, and obtain a Level One Fingerprint Clearance Card.
