CLINICAL PROGRAM MANAGER III

POSITION SUMMARY

Under the general direction of the Bridging Families program director and requiring independent judgment in the application of established procedures, develops and manages the treatment plan for children and families who are receiving Bridging Families program services. Job performance contributes to the achievement of the goals of the Bridging Families Program. Performance of the typical responsibilities and duties requires the ability to explain to/instruct and negotiate with others, involving job-related matters generally of unusual complexity.

TYPICAL RESPONSIBILITES AND DUTIES:

Works directly with Bridging Families staff providing supervision and support for the on-going care of clients and their families. Provides accurate and timely record keeping, data collection, and sharing of information such as meeting minutes and CANS summaries. Data collection includes information from assessment tools such as but may not be limited to CANS, PSC-35, CSQ-8, and Consumer Perception Survey. Provides guidance in the use of outcome measurement tools, Mental Health Assessment, Problem Checklists, and Treatment/Care Plans as well as the utilization of trauma-informed best practices used by all direct service personnel Demonstrates a high level of clinical competence regarding care of children and families. Demonstrates a high level of competence with the Integrated Core Practice Model and its application to serving children and families receiving Bridging Families services. Oversees quality, culturally proficient, LBGTQIA inclusive and relevant therapeutic, educational, preventative and crisis services to children and their families in community-based and in-home settings. Provides active assistance to and oversight of Child and Family Teams. Works collaboratively with other agency staff and with individuals and agencies in the community to ensure relevance, accessibility, and quality of service provided to children and their families. Participates in the development and implementation of such collaborative partnering. Provides staff training and supervision on an on-going basis. Documents necessary information as required within the Bridging Families program. This includes client documentation including Coordinated Care Documents, Incident Reports, and other report writing. Assists in the development and refinement of existing documentation and data collection systems. Documents charts according to the State and County MediCal requirements. Attends trainings by Santa Clara County in Medi-Cal documentation. Maintains professional standards regarding client rights. Demonstrates an understanding of confidentiality policies, mandated child abuse reporting and duty to warn procedures. Utilizes clinical supervision in any case where these issues are in question. Demonstrates the ability to think creatively and intervene/respond in crisis situations. Utilizes supervision, consultation and training as necessary to continue to provide high-quality services to children and their families as effectively and efficiently as possible. Every employee has the opportunity and responsibility to participate in one or more activities each year that support the Performance Quality Improvement (PQI) of RCS programs and services. Activities might involve membership on a PQI committee or work group, service as a peer reviewer, data collection for PQI and evaluation purposes, completion of PQI questionnaires and surveys or participation in focus groups, or other activities as identified by PQI committees or work groups. Provide clinical supervision in compliance with the BBS (or identified licensing board) and SCC standards of practice

MINIMUM QUALIFICATIONS:

EDUCATION, TRAINING AND EXPERIENCE:

Licensed MSW/MFT/LPCC or equivalent with two years post licensure experience. Three (3) years of experience in a setting working with children and families. Experience working in a children’s mental health setting emphasizing a family systems perspective preferred. Demonstrated experience working collaboratively with public child-serving systems, including Child Welfare/DFCS and Juvenile Probation. Experience developing and maintaining cross-system partnerships and coordinating services for children and families involved with multiple systems strongly preferred. Supervisory and management experience. High degree of familiarity and proven experience with principles, application to service delivery, and best practices. Training experience preferred.

LICENSE(S):

California license (LCSW, LMFT, LPCC, or PhD) required. California driver’s license (and good DMV report) Licensed post two years and eligible to provide clinical supervision.

KNOWLEDGE AND SKILLS:

Strong skills in cross-system collaboration, relationship building, conflict resolution, and facilitating multidisciplinary teams involving families, county partners, and community-based providers. Advanced skills in brief, cognitive, behavioral therapy, permanency, and trauma informed service. Advanced skills in crisis intervention, suicide assessment, conflict resolution, interpersonal communication, clinical supervision and team management. Ability to maintain a positive perspective in the treatment and care of emotionally disturbed youth and their families. Knowledge of principles of developmental psychology, including physical, mental, emotional maturation pattern, and special needs and problems of adolescents and younger children, basic learning, communication, interviewing, and counseling techniques. Ability to develop and maintain a creative and optimistic spirit

SPECIAL REQUIREMENTS:

Must be able to meet and receive a criminal records clearance as required by Title XXII licensing regulations.

PHYSICAL AND MENTAL REQUIREMENTS:

The following are required in day-to-day performance of the duties of this position: Sitting, walking, standing, grasping (simple), and visual/auditory acuity are constantly required (over 2/3 of the workday). Bending, squatting, kneeling, twisting, lifting, reaching and pushing/pulling are required occasionally (under 1/3 of the workday).
