Case Manager

We believe quality, affordable housing is the foundation for prosperity, personal fulfillment, and success. WHAT WILL I BE DOING? CHI is looking for a full-time Case Manager for our Coram location. You will be responsible for ensuring that all program participants receive the services necessary to promote personal and economic independence, develop the skills required to obtain, maintain, and retain permanent housing, and live in well-maintained units. Your key duties include: Adhere to and enforce program regulations, policies, and procedures, ensuring clients receive quality services that follow program requirements. Assist clients in developing an Independent Living Plan (ILP) by identifying needs and goals, and evaluating client progress on short- and long-term goals. Provide information, referrals, counseling, crisis intervention, direct service, and advocacy services for all clients. Develop and maintain a resource network for referrals related to the needs of the individuals being served, collaborating with agencies such as DSS to assist clients in fulfilling ILP and program requirements. Assist clients with direct services such as current and projected budget, resume assistance, and ADL skill support. Maintain face-to-face meetings with clients. Inspect living space regularly to assess ADL needs and program compliance, helping clients maintain housing. Document and limit inappropriate use of agency resources by clients, such as unit damages, furniture, or appliances. Evaluate, document, and inform the direct supervisor of physical plant needs for housing units or common areas. Complete discharge plans with clients. Submit all internal and external reports promptly, including housing logs, unit inspection sheets, progress notes, ILPs, authorization forms required by DSS, and special projects. Maintain charts and files in an orderly fashion. WHAT DO I NEED? Education: Bachelor’s Degree in human services or related field; Master’s Degree in Social Work preferred. Extensive experience may substitute for education. Experience: Two years of experience in a related field such as homelessness or housing preferred. Communication: Excellent verbal and written communication skills. Proficiency in English required; bilingual (Spanish) is a plus. Computer Skills: Basic competency in MS Windows, MS Office, and internet usage. Math Skills: Ability to formulate simple financial budgets. Physical Performance: Ability to tour property, walk distances, and climb stairs. Reasoning Ability: Ability to prioritize, make appropriate decisions, and exercise judgment. Other Skills: Ability to relate to underserved, homeless, and disabled populations; work independently and as part of a team; be highly organized; assume responsibility; work well under pressure; meet deadlines; multitask; be flexible; and maintain a high level of attention to detail. ANYTHING ELSE? Salary: Bachelor’s degree: $24.04/hr ($50,000 annually). Master’s degree: $26.45/hr ($55,000 annually). Overtime available. Open shifts: Sunday - Thursday, 10 a.m. - 6 p.m. WHY CHI?
