RN Care ManagerOhio's Hospice is seeking a compassionate and dedicated Registered Nurse (RN) Care Manager to provide high-quality hospice care to patients and families throughout Franklin County.This is a full-time, field-based nursing position requiring local travel to patient homes and care settings.Schedule:40 hours per weekMonday–Friday, 8:00 AM–4:30 PMWeekend rotation approximately every 6 weeksTwo holidays per yearThe RN Care Manager works closely with patients, families, caregivers, physicians, and members of the interdisciplinary hospice team.
