Social Services Director Village Green – Greenville, OH Make a Difference Beyond the Care Plan. Village Green is looking for a compassionate, organized, and resident-focused Social Services Director to join our team. This position plays a vital role in advocating for our residents, supporting families, and helping each resident feel heard, valued, and at home. What You'll Do: Serve as an advocate for residents and help ensure their psychosocial needs are met Complete social services assessments and develop individualized care plans Provide emotional support and assistance to residents and their families Coordinate and participate in care conferences Help residents adjust to their new environment and changes in their care needs Address resident and family concerns and work toward positive resolutions Maintain accurate and timely documentation in accordance with regulatory requirements Assist residents and families with discharge planning and community resources Collaborate with nursing, activities, therapy, dietary, and other departments as part of the interdisciplinary team Support resident rights, dignity, choice, and quality of life Participate in facility programs, meetings, and quality improvement initiatives What We're Looking For: Experience in social services, healthcare, long-term care, or a related field preferred Strong communication and interpersonal skills Compassionate and empathetic approach Excellent organizational and problem-solving abilities Ability to work effectively with residents, families, and an interdisciplinary team Knowledge of long-term care regulations and resident rights is a plus Ability to maintain confidentiality and handle sensitive situations professionally Why Join Village Green?
