Social Services Director Milton Freewater Health and Rehabilitation Center Milton-Freewater, OR | Full-Time $24.00 - $28.00 per hour Milton Freewater Health and Rehabilitation Center is seeking a compassionate and dedicated Social Services Director to join our skilled nursing team. In this role, you will support residents and families, coordinate discharge planning, conduct psychosocial assessments, advocate for resident rights, and collaborate with the interdisciplinary care team to ensure exceptional resident care and quality of life. Qualifications: Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or related field Previous experience in a Skilled Nursing Facility or healthcare setting Knowledge of resident rights, care planning, and discharge planning Strong communication, organizational, and interpersonal skills Responsibilities: Complete psychosocial assessments and care plans Coordinate resident discharge and transitional care services Provide support and resources to residents and families Participate in care conferences and interdisciplinary team meetings Ensure compliance with federal and state regulations Advocate for resident well-being and quality of life Benefits: $24.00 - $28.00 per hour Medical, Dental, and Vision Insurance Paid Time Off 401(k) Professional Growth Opportunities If you are passionate about making a positive difference in the lives of seniors and their families, we would love to hear from you!
