Responsibilities include: Oversee and direct social services operations within the facility Conduct psychosocial assessments and maintain required documentation Participate in care planning and provide appropriate input for MDS assessments Advocate for residents' emotional, social, and mental well-being Collaborate with the interdisciplinary team during care conferences and care planning Assist residents and families with discharge planning, transfers, and accessing community resources Build positive relationships with residents, families, and community partners Support smooth transitions of care and discharge planning Maintain compliance with applicable state and federal regulations Help support resident engagement and quality-of-life initiatives Provide guidance and leadership within the social services department Who We're Looking For Bachelor's degree in social work or a related field preferred Current Colorado social work licensure or eligibility preferred Previous experience in skilled nursing, rehabilitation, or long-term care strongly preferred Knowledge of MDS, care planning, and regulatory requirements preferred Strong communication, organizational, and leadership skills Compassionate and committed to resident advocacy and person-centered care Ability to work collaboratively with residents, families, and an interdisciplinary healthcare team Why This Role Matters As the Social Services Director, you'll have the opportunity to build meaningful relationships with residents and families while helping ensure their emotional and social needs are supported.
