Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on the creation of a caring environment that promotes healing and wellness. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Coordinating, ensuring, and providing direct patient care activities on the unit includes: - Provision of nursing rounds, assessments, and treatments. - Facilitating admission, transfer, and discharge activities. - Provision of patient and family-centered education. - Care coordination and individualized treatment planning as applicable. The RN is responsible for continuous assessment of patient needs and recognizes, responds to, and communicates changes in patient condition. The RN conducts and documents ongoing assessments and responses to procedures and therapies, using nursing processes according to established standards. The RN delegates tasks appropriately and oversees team members in the performance of activities regarding patient care. The RN identifies and addresses educational needs of patients related to prioritized problems according to their level of comprehension. The RN facilitates the patient discharge process by executing discharge orders in a timely manner, assuring the performance of discharge teaching, and coordinating the plan with Case Management and others as indicated. The RN actively participates in unit-based activities in support of organizational values and priorities. Committing to a holistic perspective regarding patient care is essential for this role.
